Initial Assessment We perform a thorough evaluation of the aging client. The assessment
focuses on the client’s
medical condition, cognitive abilities, review of medications, overall safety
evaluation and emotional and socialization needs.
Consultation and Care Planning Consultations help
adult children understand the aging process.
We outline the elder’s needs and identify ways to meet these needs. We develop a short and long term plan that
provides clients with the highest quality of care while preserving independence and dignity.
Arranging for Service Providers At your request, we will arrange for
caregivers and any other service providers
to meet the needs of your loved one. We
have the experience and practical knowledge to ensure that a caregiver has the
right skill set for optimal care.
Monitoring When appropriate, we regularly visit the client and maintain
ongoing contact with the healthcare providers and caregivers to ensure that
changes in the client’s condition are addressed promptly.
Coordination We ensure sharing and coordination of
medical information between the client, doctors, specialists and family
members. We refer to ourselves as “The
Quarterback of the Medical Team.”
Communication We regularly update the family members as to the status of
our efforts and any concerns that may arise.
We make recommendations for changes in approach as required. We will also ensure that the important legal documents such as living wills and medical
and financial powers of attorney are in place or refer you to an Elder Law
attorney.
