We take a multi-step approach to our work:

Initial Assessment
We perform a thorough evaluation of the aging client. The assessment focuses on the client’s medical condition, cognitive abilities, review of medications, overall safety evaluation and emotional and socialization needs.

Consultation and Care Planning  Consultations help adult children understand the aging process.  We outline the elder’s needs and identify ways to meet these needs.  We develop a short and long term plan that provides clients with the highest quality of care while preserving independence and dignity.

Arranging for Service Providers  At your request, we will arrange for caregivers and any other service providers to meet the needs of your loved one.  We have the experience and practical knowledge to ensure that a caregiver has the right skill set for optimal care.

Monitoring   When appropriate, we regularly visit the client and maintain ongoing contact with the healthcare providers and caregivers to ensure that changes in the client’s condition are addressed promptly.  

Coordination   
We ensure sharing and coordination of medical information between the client, doctors, specialists and family members.  We refer to ourselves as “The Quarterback of the Medical Team.”  

Communication 
We regularly update the family members as to the status of our efforts and any concerns that may arise.  We make recommendations for changes in approach as required.  We will also ensure that the important  legal documents such as living wills and medical and financial powers of attorney are in place or refer you to an Elder Law attorney.

Nancy L. Carey, RN, BSN
Managing Director

Nancy Carey, BSN is the Managing Director of The Geriatric Care Consultants and is a Certified Dementia Practitioner.   She has many years of nursing experience including 10+ years as a Home Health Nurse and Diabetes specialist, Intake Coordinator for a Home Care Servicer and 17+ years as a Geriatric Care Manager.  She has enjoyed working as a volunteer with elderly groups and ran a stroke support group.  She served on the Lake Forest Lake Bluff (Illinois) Senior Resources Commission from 2010-2016. 

She holds a Bachelor of Science degree in Nursing from Russell Sage College in Troy, NY and is a member of the Aging Life Care Association (formerly National Association of Professional Geriatric Care Managers).  http://aginglifecare.org
                                 
Amy Paulus,  MSN, APRN, FNP-BC
Care Manager

Amy joined the team in 2008. Amy is a Board Certified Family Nurse Practitioner and also is a Certified Dementia Practitioner with over 19 years of experience. She has a strong clinical background with experience at Regency Medical Center in Barrington, IL and has multiple years of experience at both Lake Forest Hospital and Good Shepherd Hospital in Barrington, IL where she currently works in the Health Management Department. Amy’s focus is on disease prevention and health promotion. She is active with the Barrington Area Council on Aging (BACOA), a member of the American Nurses Association (ANA) and the American Association of Nurse Practitioners (AANP).

Lorena Nehring-Shapiro, LPN, MS
Case Manager

Lorena joined the GCC team in 2011 and has a Masters Degree in Counseling.  She has multiple years of experience in hospitals, home care and assisted living facilities.  She has worked at the NSSC/Evanston Senior Center.  She has co-facilitated groups for individuals deals with Alzheimer's and Parkinson's disease.
